Phase III: Support plan implementation and progress tracking

Tracking Progress

Creating a support plan is just the beginning of the process. The support team must work together with the learner to implement the plan and the learner's progress needs to be tracked.
  • Record the learner's progress using the Tracking template, which can be downloaded from the dropdown template menu at the top of the page.
  • From the support plan you developed, copy and paste the Issues Identified, Learning Objectives, Learning Strategies, and Desired Outcome into the relevant place in the Tracking template.
  • Then, at the end of each week indicate if and how many times the learning strategies were used by the support team. This provides a record of the steps taken to help the resident achieve the learning objectives identified in the support plan.

Formative Evaluation

At the mid-point and at the end of the support plan period, the learner's progress should be evaluated.
  • Evaluate the learner using the Mid-Point Evaluation and Final Evaluation forms, which can be downloaded from the dropdown template menu at the top of the page.
  • Copy and paste the Issues Identified, Learning Objectives, and Desired Outcome from the support plan
    into the evaluation form.
  • Then, rate the learner's progress towards each of the learning objectives (i.e., no improvement, minimal progress, progressing well).
  • You can also make comments regarding the learner's progress towards each of the learning objectives.
  • These evaluations should be discussed with the resident face-to-face, as well as with the support team.
  • Both you and the learner should sign the evaluation to indicate agreement.
  • The learner should be offered the opportunity to include his or her comments as desired.

Summative Evaluation

At the end of the rotation the learner's overall progress is evaluated. This is Phase IV of the Academic Support Process.
